  • COMPLETED APPLICATION

  • MONEY

  • PHOTO

  • COPY OF CLUB CARDS

  • COPY OF PROVINCIAL LICENCE

  • COPY OF PAST COMPETITION LICENCE

  • RACE RESULTS OR DRIVER LOG BOOK SHOWING MOST RECENT RESULTS

  • AND APPROPRIATE MEDICAL FORM.
