Is there a specific medical form for the medical exam or do you just need a letter from a physician stating eg this person is both mentally and physically fit to drive a race car ?

After having done having completed the Driving certificate can one go ahead and enter novice races, or is it mandatory to attend a Practice session first?

I believe the process requires you to complete your driver training, to get your certificate, then get a Novice Race License.  Once you have the Novice Race License, you can enter a novice race without any further mandatory practice or training.  Novice races are similar to regular races in that you get one practice session the morning of race day, and one qualifying session on track, followed by the actual race.  Novices are monitored while on track and sometimes given special flag situations to test you.  Novices also must report to the Novice director at the track at the beginning and end of race day.

Getting a Novice Race license involves filling out applications for CACC and providing the medical forms and your school certificate.  Oh, and they want a bit of money too.  Its all done through the CACC website.
