I'm just working my way through the online License application form and I'm wondering about the medical form(s).

There is a form for my physician to fill out,  but the application only seems to require submission of the Self-Declaration.

Do I actually need to submit the "Medical Form" or do just submit the "Self-Declaration"?

You must submit a medical form once every 2 years, until you reach a certain age....OLD....like dirt....., then it's every year.   ;D

So because this is your first license, you must submit it.

Additionally, if Gayle reads your post, you could be in for a loooong racing season :-X ;D.

The ORIGINAL copy of the medical form from your doctor is required to be mailed/give to the Race Licence Administrator (me). Attaching a copy to the On Line form is not required.
