CACC License Application Form Question

I'm just working my way through the online License application form and I'm wondering about the medical form(s).

There is a form for my physician to fill out, but the application only seems to require submission of the Self-Declaration.

Do I actually need to submit the "Medical Form" or do just submit the "Self-Declaration"?

Posts: 166	Re: CACC License Application Form Question Reply #1 - Jan 12^th, 2012 at 9:55am You must submit a medical form once every 2 years, until you reach a certain age....OLD....like dirt....., then it's every year. So because this is your first license, you must submit it. Steve

Re: CACC License Application Form Question Reply #3 - Jan 12^th, 2012 at 3:57pm The ORIGINAL copy of the medical form from your doctor is required to be mailed/give to the Race Licence Administrator (me). Attaching a copy to the On Line form is not required.
